Full-Time: 80-hrs per pay period

Hours:  8:00am - 4:30pm

Position Summary:

Responsible for assessing patient and family psychosocial needs and establishing appropriate care plans including counseling, advocacy, information/education, discharge planning, emotional support, financial counseling and agency/institutional referrals.The Social Worker provides social work services on assigned units.Responsibilities shall be discharged in such a manner as to support the goals and objectives of the Medical Center’s Social Service program and in keeping with the Philosophy and Mission of the Sisters of St. Joseph of Carondelet.

The Social Worker must demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served in his/her department.The individual must acquire and/or demonstrate knowledge of the principles of growth and development over the life span.He/she must be able to provide care with consideration of developmental status.

Primary (Essential) Duties:

  1. Services to patients and their families.
    • Assess psychosocial needs of patients and their families with attention to the psychological, environmental and financial impact of illness.
    • Evaluate psychosocial situations and problem areas as presented by patients and/or family.  
    • With patient and/or family, establish treatment goals and provide alternatives to assist in attaining these goals.
    • Collaborate with other members of the health care team in the development of plans to address identified psychosocial needs.
    • Maintain timely and complete documentation of initial evaluation/psychosocial assessments of patient and family situation.
    • Actively support a system to ensure timely and efficient identification of patients who require discharge planning.
    • Provide social work intervention to patients and their family in relation to:
      • Emotional problems including assisting the patient and family with anxieties and stress precipitated by illness, difficulty in coping with residual physical disability, fears related to helplessness and death, counseling concerning need for institutional and specialized care.
      • Social and practical discharge planning issues including seeking financial aid, securing a homemaker, arranging for home care services and equipment, collaboration with community agencies, referrals to other community agencies for specialized help and determining need for legal assistance with power of attorney, etc.
    • Maintain timely and complete documentation of social work assessments, ongoing interventions and outcome of intervention as delineated by social service policy and standards.
  2. Development and maintenance of liaisons with medical and hospital staff, as well as community agencies.
    • Provide consultation to members of medical and hospital staff and community agencies in efforts to provide options for patients and families.
    • Participate in community social welfare organization and planning activities as related to the interest of patients, families, and the Medical Center.
  3. Support functions and activities of the Cancer Center Department.
    • Know and practice the philosophy, purpose, policies and standards of the Cancer CenterDepartment and the Medical Center.
    • Assist with quality assurance monitoring and evaluating the effectiveness of social work activities.
    • Protect confidentiality of all information and respect patient and family rights.
    • Participate in committee activities as assigned.
    • Participate in staff and in-service educational meetings.

Qualifications

Required:

  • Bachelor's degree in Social Work
  • Must be currently licensed by the State of Idaho to practice social work or eligible for licensure within 90 days of hire date, and maintain licensure
  • Minimum of one-year social work experience required. 

Preferred:

  • Master's degree in Social Work
