Job Summary

  • The Licensed Practical Nurse, Dialysis performs dialysis and patient care activities for the End Stage Renal Disease and Acute Care patient under the supervision of a Registered Nurse in accordance with Kindred Healthcare policies, procedures, and guidelines, as well as OSHA, JACHO, CMS, federal, state, and local regulations. This position reports to the Dialysis Program Manager or Dialysis Supervisor at the respective facility

Essential Functions 

  • Performs patient assessment of vital signs and all other parameters of ESRD patients
  • Reviews dialysis orders, notes general physical and mental condition of patients through observations and interviews
  • Provides data to determine observations and interviews, provides data to determine compliance to dietary or medication regimen; documents observations appropriately
  • Collects patient data including but not limited to assessing and monitoring status of vascular accesses, reviews lab work and patient assessments; seeks guidance when levels are out of “normal” range for ESRD patients
  • Knows and complies with Kindred policies and procedures in accordance with the state and federal regulations for patient care and services (Medicare, OSHA, HIPAA, or other governing policies)
  • Provides acute dialysis therapy within the scope of practice and according to the plan of care; recommends a dialysis plan for ultra-filtration; initiates dialysis via various accesses, complies with all safety measures
  • Monitors patient vital signs applicable to ESRD patients
  • Administers medications per physician instruction if allowed by state agencies, fluid therapy and blood products or other appropriate treatments as needed, documents activity appropriately
  • Coordinates and oversees work of Dialysis Technicians

Knowledge/Skills/Abilities/Expectations

  • Knowledge of the Hemodialysis and Peritoneal Dialysis Process
  • Demonstrated ability to lead and oversee the dialysis process performed by Dialysis Technicians
  • Demonstrated ability to assist with providing quality outcomes and tracking of metrics
  • Demonstrated ability to provide treatments in a safe, therapeutic and cost-effective manner
  • Functional proficiency in all computer systems of Kindred within 90 days of hire required
  • Approximate percent of time required to travel: 100% 
  • Must read, write and speak fluent English
  • Must have good and regular attendance
  • Performs other related duties as assigned

Qualifications

Education

  • Graduate of an accredited program for licensed vocational/practical nurses

Licenses/Certifications

  • Current LPN – LVN license in the state of practice required

Experience

  • More than 12 months of clinical nursing experience
  • Twelve months providing nursing care to dialysis patients
  • Basic computer skills and proficiency in MS Word and Outlook
